The Leapfrog Group was formed by The Business
Roundtable and numerous fortune 500 companies
in response to a 1999 study by the Institute
of Medicine that reported thousands of Americans
die each year from preventable medical errors.
The Leapfrog Group launched "a national
effort to reward hospitals for advances in patient
safety and to educate the public about the importance
of hospitals' efforts in this area."
Today, the Leapfrog Group is a national coalition
whose goal is to improve the patient safety,
healthcare quality and overall value. The Leapfrog
Group includes over 135 public and private purchasers
who have agreed to specific purchasing principles
to drive improvements in healthcare. The Leapfrog
Group has worked with patient safety experts
to identify three initial patient safety standards
that serve as the focus of provider performance
comparisons and hospital recognition and reward
efforts.
The standards include: computerized
physician order entry, intensive care unit physician
staffing, and evidence-based hospital referrals. |

mistake. However, no specific representation

at these hospitals. A hospital's placement
in a quartile does not convey whether
its risk-adjusted mortality rate is statistically
significantly different from (a) the
statewide average or (b) the risk-adjusted
mortality rate of any hospital in its
quartile or any other quartile. |
