Patient Identification and Eligibility

All ECOH enrollees are issued an identification card with either ECOH Network Plan or ECOH River Valley clearly stated on the front. The identification card will provide information on co-pays, third party administration, utilization management, precertification, as well as claim submission. To verify eligibility, please refer to the TPA phone number on the back of the identification card.

PLEASE BE SURE TO ASK THE PATIENT FOR HIS OR HER ID CARD AND MAKE A COPY, FRONT AND BACK FOR FUTURE REFERENCE.

(Note: For January 1, 2008, all enrollees were issued new ID cards containing new ECOH claims address information.)
