Where do I send Claim Forms?

All ECOH claims (both Network and River Valley) are sent to:

Who do I contact to determine the status of a claim?

Call the ECOH Repricing Team at 800-990-3204 or fax at 815-227-5661.

Who do I contact if a claim was processed incorrectly?

Contact ECOH at 815-397-0790 as the first step. We will determine if the claim was repriced correctly and then may refer you to the appropriate Third Party Administrator.

Who do I contact to submit or determine the status of a referral?

Who do I contact if we have a change of address, tax identification number or have added additional providers to our office?

Fax this information to the attention of the Member Services Director at ECOH at 815-397-2790. Your fax should include the effective date of the change.
