Where do I send Claim Forms?

ECOH claims (both Network and River Valley) are sent to:

For Providers contracted with HSM:

Chiropractic and Physical Therapy claims are sent to:

Who do I contact to determine the status of a claim?

Please check the ECOH Claim Status Web Portal. Please click here for information on how to obtain a Web Portal account.

You may also call the ECOH Repricing Team at 800-990-3204 or fax at 815-397-2790.

Who do I contact if a claim was processed incorrectly?

Contact ECOH at 800-990-3204 as the first step. We will determine if the claim was repriced correctly and then may refer you to the appropriate Third Party Administrator.

Who do I contact to submit or determine the status of a referral?

Who do I contact to determine patient benefits and eligibility?

You will need to contact the Third Party Administrator for the patient's employer. Phone and/or website information for the Third Party Administrator (TPA) is found on the back of the patient's health plan ID card ("For benefit and claims information please contact..."). You may also check the Plan Information page of the ECOH website to find TPA information for each Member Company.

Who do I contact for fee schedule rates?

Call the ECOH office at 815-397-0790 x3901.

Who do I contact if we have a change of address, tax identification number or have added additional providers to our office?

Fax this information to the attention of the Director of Provider Services at ECOH at 815-397-2790. Your fax should include the effective date of the change and provider NPI number.
