Claim Status Web Portal
Providers can check claim status via the ECOH Claim Status Web Portal.

If you do not yet have a Claim Status Web Portal account, please download and complete the ECOH Web Portal Access Request Form and an ECOH staff member will contact you regarding setting up your account.

Claims Submission
All physician claims should be submitted on the HCFA
1500 form and all hospital claims should be submitted
on the UB-92 form. When submitting claims, please use
the appropriate coding (CPT-4, Revenue Code, DRGs, ICD-9
procedure or diagnostic codes).
Please include the following on all claims:
- Provider Name, Tax ID Number, NPI Number, Address, Phone Number
- Patient Name
- Name and Social Security Number of Insured
- Appropriate CPT, ICD-9 or DRG Code
