ECOH Medical Home is a new primary care program model that delivers unlimited, comprehensive primary care through Swedish American family medicine physicians, at Partners Health Center (PHC) in Rockford, IL.

With an emphasis on achieving the most healthy lifestyle attainable as defined by physician and patient working together, ECOH Medical Home provides a broad range of health care and health support services including labs, limited pharmacy, wellness coaching and broad patient support at the PHC facility.
