Patient Identification and Eligibility

All ECOH enrollees are issued an identification card. ECOH Network Plan ID cards are white and ECOH River Valley ID cards are blue. The identification card will provide information on co-pays, third party administration, utilization management, precertification, as well as claim submission. To verify eligibility, please refer to the TPA phone number on the back of the identification card.
