To confirm your eligibility and to determine what services are covered and at what levels, you will need to contact your Third Party Administrator or insurance company.

Phone number and/or web address information for your Third Party Administrator or insurance company, Prescription Benefit Manager, Out-of-Area Network, and pre-certification information are found on your health plan ID card.

If you are traveling out of the immediate ECOH area (northern IL, southern WI), and need a provider, please contact your Third Party Administrator. The name and phone number for your Third Party Administrator  ("to obtain specific benefit and claims information please contact...") can be found on the back of your health plan ID card (please see image above).
